Lost Cards and How to Replace Them
Lost or Stolen ID Card
It is important to immediately report a lost or stolen University ID card to protect your accounts and privileges.
There are two steps to report a missing card and replace it:
- Go online to deactivate immediately.
- Submit an application and schedule an appointment for a new ID card.
You are responsible for any account activity before reporting your card's loss. If you find your lost card and haven't replaced it with a new one, you may easily reactivate it the same way.
If you find someone else's ID card, please notify the ID Card Office and return the ID card to the Academic ID Office.
How to Replace an ID Card
A $20 fee, payable via Cavalier Advantage, is required to replace lost or damaged cards and is due at pickup. Cards replaced due to a malfunction or a name change are issued at no cost, provided the previous card is returned at the time of service.
The same documentation presented when receiving your first ID Card is required for application and pickup, a valid government-issued photo ID.
Acceptable forms of ID include:
- U.S. Driver’s License
- U.S. Identification Card
- U.S. Permanent Resident/U.S. Department of Homeland Security/U.S. Green Card
- Passport Book
- U.S. Military ID Card
Only the card owner can pick up the card, and ID cards are never mailed.
