Health System Employee & Student ID Card
Employees and students of the Medical Center, University Physicians Group, School of Medicine, and Nursing are required to wear a UVA Health ID Badge. To expedite the process of obtaining your UVA Health ID Badge, upload your photo below the week before visiting the UVA Health ID Badge Office. When picking up your badge, you will need your 9-digit employee number (displayed on the right side of your Workday profile page) and a photo ID (i.e. driver’s license).
Those requiring authorized access to UVA Health Buildings (Academic employees in student health, police, security, facilities management, and select other departments) can contact the UVA Health ID Badge Office.
The UVA Health ID Badge Office is located at 1335 Lee Street, Room 1205, located in the front corridor of the West Complex Multistory Building. Questions? Call 434-982-4009 or email for more information.
Hours of operation are 8:30 am to 4:15 pm, Monday-Friday.
If you lose your UVA Health ID Badge, please contact the UVA Health ID Badge Office.
Note: Persons requiring access to the UVA Health and Academic card readers are only issued a UVA Health ID Badge. The UVA Health ID badge provides access to both the UVA Health and Academic card readers.
What Do I Need?
A valid government-issued photo identification is required to obtain your UVA Health ID badge.

Note: Driver's License's stamped "void" and Passport "cards" cannot be accepted. See here for Information on obtaining a U.S. Driver's License or U.S. Photo Identification card from DMV.
Important: The name on your government-issued photo identification must match the name in the University's database exactly.
